CPRS DOCUMENTATION TOOL
The CPRS Documentation Tool is the authorized Electronic Health Record
(CPRS) corrections tool, utilized for requesting corrections at MEDVAMC.
All requests for corrections in the Electronic Health Record (CPRS) must be
done using this tool. The tool is easily accessed from within CPRS while on the
error in question.
Requests done through the CPRS Documentation tool are sent to the
corrections department utilizing a VistA Mailman message. All requesters are
responsible for ensuring that their requests were processed. A VistA Mailman
reply will be sent to the requester's account within two business days. If a
reply is not sent within two business days, send an email to VHAHOU CACS.
Requesters should refer to this tutorial on how to appropriately enter
a request. If further questions arise send an email to VHAHOU CACS.

RETRACT DOCUMENT
When a document is placed in retracted status, it is no longer viewable by general CPRS
users. Prior to sending a retraction request, an addendum MUST be attached to the note, stating
that the note was entered in error or entered on the wrong patient.

RE-LINK TO VISIT
A note erroneously linked to the wrong visit/appointment needs to be corrected by sending this
CPRS DOCUMENTATION Request.

CONSULT LINK
To close out a consult request, the consulting provider must write a note with the word Consult in
the Title. If this is not done at time of note writing, the provider can request this be corrected using
Consult Link.

IMAGE DELETION
Requesting images to be deleted should be done with this feature. Make sure to provide the image
number(s) of the image to be deleted and enter clear and complete information about your
request.

REMOVE DISCHARGE SUMMARY ADDITIONAL SIGNER
Only authors of a discharge summary may remove additional signers from the discharge summary.
If the author added your name erroneously and they are not available to make a correction, use
the Remove Additional Signer option on the CPRS DOCUMENTATION Tool to request an edit.

CHANGE TITLE
Requesting to change a note title is used with this feature.

OTHER REQUEST
All requests that are not from one of the above categories should be entered using the Other
Request feature. Make sure to enter clear and complete information about your request.
